you need to give him one more chance and let him know that he won't be getting another one. tell him if he breaks the contract that her name will immediately be changed. if he follows it then you keep her name the same. and unless her name is legally changed then she will always be known by his last name, especially at places like doctors offices. good luck & congrats on the marriage.
